A CareCheck registered nurse visits and builds the plan with you. If you're the paid caregiver, training is scheduled around your life.
- Home visit: about an hour, on your schedule.
- Training: in chunks that fit your week.
- Paperwork: filed by us.

Within 2–3 weeksStart services
Once the state approves, services begin: monthly tax-free deposits if you're the caregiver, scheduled aide visits if not.
- SFC: up to $1,800/month, tax-free, monthly.
- HHA: up to $2,500/month of Medicaid-covered care.
- Both: ongoing nurse-coach check-ins.

We help you secure a referral from a PCP, social worker, or your local Area Agency on Aging (AAA). This part moves at the state's pace.
- From: PCP, social worker, or AAA.
- Triggers: the state's formal assessment.

2–4 weeksState assessment
An Ohio Medicaid assessor visits to determine level of care. We handle scheduling and paperwork.
- A home visit from the AAA assessor.
- Result: approved level of care opens the waiver.

Within a week of approvalCare plan
A CareCheck registered nurse builds the plan with you. Caregiver training scheduled around your life.
- Home visit: about an hour.

Once the waiver is openStart services
