Your timeline

Two paths to the same place: home, supported, paid.

Already on PASSPORT, MyCare Ohio, or the Ohio Home Care Waiver: 4–6 weeks. Starting from scratch: longer.

    Check eligibility

    Answer a few questions online. We confirm waiver status and tell you on the spot which program fits.

    • We ask: waiver status, care needs, your relationship.
    • You get: a clear yes / probably / not yet.

    Within 1–2 weeks

    Care plan

    A CareCheck registered nurse visits and builds the plan with you. If you're the paid caregiver, training is scheduled around your life.

    • Home visit: about an hour, on your schedule.
    • Training: in chunks that fit your week.
    • Paperwork: filed by us.

    Within 2–3 weeks

    Start services

    Once the state approves, services begin: monthly tax-free deposits if you're the caregiver, scheduled aide visits if not.

    • SFC: up to $1,800/month, tax-free, monthly.
    • HHA: up to $2,500/month of Medicaid-covered care.
    • Both: ongoing nurse-coach check-ins.

    Referral

    We help you secure a referral from a PCP, social worker, or your local Area Agency on Aging (AAA). This part moves at the state's pace.

    • From: PCP, social worker, or AAA.
    • Triggers: the state's formal assessment.

    2–4 weeks

    State assessment

    An Ohio Medicaid assessor visits to determine level of care. We handle scheduling and paperwork.

    • A home visit from the AAA assessor.
    • Result: approved level of care opens the waiver.

    Start services
