How a referral moves

Four steps from your introduction to first service.

    You introduce us.

    Email [email protected] or call (380) 250-7821. We confirm receipt same business day.

  2. 02

    We reach out to the family.

    A care specialist contacts them within one business day. You get a status note when we make contact.

  3. 03

    RN home visit and care plan.

    An RN visits and builds the plan. We file the paperwork; the family signs.

  4. 04

    Services begin.

    4–6 weeks if already on a waiver. We send a closing note when first service is delivered.

What we need from you

The minimum to start.

  • The family's name and a working contact (with their permission).
  • One sentence on the situation.
  • Any urgency (active discharge, scheduled assessment, etc.).

No clinical records, diagnoses, or Medicaid IDs needed from you. We collect those directly with the family's consent.

What we handle

End-to-end Ohio Medicaid enrollment.

  • Waiver confirmation and AAA coordination.
  • State-required in-home assessment.
  • RN-led care plan.
  • Caregiver training, BCII / FBI checks, HHA certification.
  • Payroll and W-2s; SFC tax-free documentation.
